This is an extremely rare--if not impossible to find--item! It is a plastic form that was used in a telephone booth to indicate that a pay phone was out of order and had been temporarily removed for repair. As its appearance suggests, it is 1960s vintage as the style of the dial and layout reflects that of the 1960s pay and desk telephones.<p>

It is exact in detail with the pay phones of that time! It is molded plastic with no manufacturer's mark. It is 19 inches long, 6 1/4 inches long, and 3 1/4 inches thick. It has labels for the dial as well as instructions on making long distance calls. I will leave it to you to re-stick the area code circle which has lost its glue. It was in a dusty storage area, so I have done my best to clean it without disturbing the printing on the labels. Notwithstanding, this is in excellent condition!
