DS, signed “Shirley Temple Black,” one page both sides, 7.5 x 3.25, February 24, 1965. Temple’s 1965 W-4 form from 20th Century Fox, filled out and signed twice by Temple, once on each side. Interesting to note she accidentally listed her birthday as 1938, instead if 1928. In fine condition, with expected punch holes and stamps.
