Bleeder is 1-1/2" x 1-1/2" x 2-1/2" at highest point, cup is 1-1/2" x 2". This is a late 1800s - early 1900 Scarificator, also known as a bleeder. This one has rounded blades, and comes with a glass cup that was used to catch the blood that came from the cut.
As long ago as the 17th century there were multi-bladed bleeders called scarificators. These became very popular in the late 18th and 19th centuries. Like spring lancets, they came in every size and description. On the top is a depth adjuster for the blades and a lever that cocked the blades, and the release switch is on the side. This allowed the blades to swing around, making multiple cuts at once. Scarificators with pointed V-shaped blades are usually pre-1820. Those with slightly rounded blades are later. Once a scarificator was used to slice and dice the patient, a cup was often placed over the wound as a receptacle for the blood.
