Document has letterhead of "United States of America // Indian Territory," but since it was used August 29, 1908, the year after statehood "Indian Territory" is overwritten "Oklahoma." Document (Form 94) is printed and filled in, measuring approximately 8.5" by 14", and bearing the embossed seal of Charles Nichols.
