The receiver on this example is engraved with the 12-5 pattern showing two dogs pointing at a grouse in a field scene on the left side and four ducks flying over a marshy area on the right side. There is also floral scroll engraved around the outside edge of the game scenes and flourishes on the trigger guard, takedown collar and rear of the barrel. There is a pigeon engraved on the bottom of the takedown collar. Listed by serial number in the book "The Greatest Hammerless Repeating Shotgun Ever Built: The Model 12 1912-1964" on page 172. This section of the book is a record of serial numbers, the engraving pattern used and the date Nick Kusmit started engraving a Winchester Model 12. The information for this serial number has the date of 6-11-55 and engraving pattern 12-5. The barrel has the two pin duck bill vent rib, is choked improved modified, has a white bead front sight and has a 2 3/4 inch chamber. The trigger and safety are gold plated with the breechblock jeweled. Mounted on a beautifully figured, Fleur-de-Lis pattern checkered, extended walnut forearm and pistol grip stock with steel grip cap, raised comb and solid red rubber Winchester recoil pad. Length of pull is 13 7/8 inches.

Very fine as partially factory refinished (receiver), 95% plus of the original blue finish remains on the balance with some high edge, takedown, cycling and handling wear overall with some scattered minor scratches. The wood is also excellent with some scattered minor scratches and dings. The recoil pad has hardened and chipped at the toe. Mechanically excellent. A beautiful example of Nick Kusmit's engraving skill!
