Partly-printed DS, one page, 8.5 x 5.5, November 12, 1958. A 20th Century-Fox Film Corporation Employee’s Personnel Statement filled out in full by Blanc and signed at the conclulsion by Blanc. In fine condition, with faint creasing, two filing punch holes at the top border, and a few office markings. RRAuction COA.
