Launched on September 25, 1992, the Mars Observer spacecraft was to conduct a global survey of the Martian surface and atmosphere. On August 21, 1993, Mars Observer was executing a sequence to pressurize the propulsion tanks in preparation for Mars Orbit Insertion three days later. As part of that sequence the transmitter was turned off, and no signal was ever detected after the transmitter was turned back on. An important report that we have not seen before.
