Each sheet has been filled out in ink or pencil by a different celebrity, using their legal names, printing their names on the top line, adding their personal information, and answering fourteen yes and no questions at the bottom. Signers are: Mitzi Gaynor, Marilyn Maxwell, William Durant, Jean Wilde (aka Jean Wallace), Barbara Bain, Jan Sterling, Hoagy Carmichael, Elsa Lanchester, Steve Allen, Liberace, Leo Durocher, Cyd Charisse, and Robert Cummings. In very good condition, with uniform toning to each sheet, as well as some folds and creases.
