The certificate is worth a maximum value of $3,980 applicable towards a laser vision correction procedure and must be presented to your counselor on the day of surgery. A pre-operative consultation will determine if the Standard LASIK or Advanced Custom Wavefront procedure is recommended. If the recipient is deemed a non-candidate for laser vision correction, the surgery may be transferred to another individual of their choosing. The recipient will also be entitled to the Vision Enhancement Plan, if any, available in the clinic where the procedure is performed at the time this certificate is redeemed. Additional costs, such as drops, are not included in the price of the procedure. Any balance between the cost of surgery and the value of the certificate cannot be reimbursed. Surgery must be completed within one year of the award date.
