International Certificate of Inoculation and Vaccination, 4 x 6, issued March 15, 1951. The front is filled out by Hepburn with her home address, birthday, and gender, and signed twice “Katharine Hepburn.” The following page bears Hepburn’s personal information, filled out in another hand, and again signed at the bottom, “Katharine Hepburn.” In very good condition, with scattered creasing (one crossing the first signature), wrinkling, scattered soiling, three small staple holes, and official stamps, one of which affects her last name of the third signature. This document was issued by the US Public Health Service to Hepburn in preparation for her journey to Africa in May 1951 to film The African Queen.
