Deer Lodge Montana Prison Intake Document, 1911
This is an original intake document from the Montana State Prison in Deer Lodge. The document was created when the convict was received at the prison. The document lists the prisoner as C. Cook, who was convicted of "forgery". Handwritten across the two accompanying photos is the convict's name and inmate number: C. Cook/ 3663. On the Intake document, the name is spelled, "Cook". He was 24 when he entered the prison on November 18th, 1911; He was sentenced to one year and six months, with release date set at March 18th, 1913 "with good time earned". The bottom portion of the document shows two I.D. photos of Cook. The document details marital status, residence of spouse, immediate family names, health, notes on marks or deformity, and other notes. The photo on the right shows him immediately prior to intake, still wearing street clothes, while the photo on the left shows him immediately after intake, with shaved head and wearing the striped inmate uniform. This is a fantastic historical Montana document. It shows good overall condition. The document measures 8 1/4" x 16 3/8"
