<B>Confederate Surgeon Dr. Samuel H. Caldwell's Appointment as Chief Surgeon to the United Confederate Veterans of Tennessee</B></I> signed by former Confederate General John Brown Gordon. Dated January 4, 1900, it is signed by the head of the UCV, former Confederate General John Brown Gordon. The document bears the gold seal of the United Confederate Veterans. There is water damage to the upper and left portion of the certificate. In gold and black calligraphy, the certificate is assigned to “Comrade Samuel H. Caldwell, MD”.
