Detailed report, filled in in another hand, states Moore has spent 23 years as a boxer, his last knockout coming in 1956, his heart rate and reports he had an appendectomy. Signed at the bottom by Moore, as well as the physician. In fine condition, with punch holes to top edge, a few creases, and a stamp to lower left.
