Flown double-sided checklist page carried into lunar orbit as part of the Apollo 13 "LM Malfunction Procedures" manual, 8 x 10.5, signed in black ballpoint, “Fred Haise, Apollo 13” and “James Lovell, Apollo 13 CDR.” This checklist page, numbered “73” and “74,” dated March 16, 1970, provides a troubleshooting flowchart for "Malfunctions, 16mm System” and “70mm Camera Malfunctions." Upper tab to the first page is marked as “16 A Thru 70 IA.” In fine condition. Accompanied by a CD-R disc and flash drive that contains full provenance and documentation for the chain of ownership.

Consignor attests to having removed this offered malfunction procedure from the original flown Flight Data File (certified as flown in its entirety by Lovell in a separate LOA).
