Questionnaire issued by St. James Press in regard to Kozintsev's inclusion in "the second edition of The Writers Directory, to be published 1st April 1973." On the two interior pages, Kozintsev marks all his responses in black felt tip, including: Surname ("Kozintsev"), Christian Name ("Grigori"), Country of Residence ("USSR"), Main Current Employment ("Film Director, Lenfilm Studio, since 1924"), and Address ("UL. BR. Vasilievykh Apt. 2 Leningrad P-46, 197046, USSR"). Under his book publishers, Kozintsev writes: "Shakespeare: Time and Conscience. Hill and Wana USA 1966, " — " Dennis Dobson. Great Britain, 1967, 'Iskusstvo'…USSR." On question 10, Kozintsev lists the headings under which he should appear in the index, "Screenplays, Art, Cinema," and then proceeds to underline them in the bottom field. All black ballpoint responses achieved by another hand. In fine condition.
